The type of personal information we collect and how we use it

At the time of your booking we will collect and process following information:

• Personal identifiers, contacts and characteristics (for example, name, date of birth, contact details and your employer (where appropriate)).

 

At the time of your appointment, we will collect and process the following:

• Personal information about you and your health (past and current), and your employment (past and current) that is relevant to your referral or medical.

• Any forms, notes or reports either provided by yourself, your employer or other medical professional. These could include your GP, a specialist consultant or previous occupational health provider.

 

The above information is used to provide our services to you in our capacity as an Occupational Health Provider.

How we get the personal information and why we have it

​

Most of the personal information we process is provided to us directly by you or from your employer.

Medical Records are managed in line with the GMC regulations and guidance. Consent for processing, storing and disclosing medical records requires consent at all stages.

 

We will not disclose your personal data, medical records or functional information or certification to any person or party other than yourself, unless there is a lawful reason to do so, without your prior consent.

 

Under the General Data Protection Regulation (GDPR), the lawful basis we rely on for processing this information is your consent. You are able to remove your consent at any time by contacting us.

How we store your personal information

 

Your information is securely stored in our UK based database. Any paper copies of any data are held in a locked cabinet in a secure premise.

Your patient data and associated medical records will be held until you request them to be removed, in accordance with the guidelines set-out by the General Medical Council (GMC) & Faculty of Occupational Medicine (FOM). Current guidance is for records to be held until ‘6 years after the end of a person’s employment or until his 75th birthday whichever is the sooner’.

Any requests for removal will be managed in line with any legal requirements for us to retain the information.

Your right of access - You have the right to ask us for copies of your personal information.

Your right to rectification - You have the right to ask us to rectify personal information you think is inaccurate. You also have the right to ask us to complete information you think is incomplete.

Your right to erasure - You have the right to ask us to erase your personal information in certain circumstances.

Your right to restriction of processing - You have the right to ask us to restrict the processing of your personal information in certain circumstances.

Your right to object to processing - You have the the right to object to the processing of your personal information in certain circumstances.

Your right to data portability - You have the right to ask that we transfer the personal information you gave us to another organisation, or to you, in certain circumstances.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
